GigE Module [MTT-50]: Rebate Form

  1. This program is only available for customers that have previously purchased an eligible product listed in the form below.
  2. To use this rebate form, you are required to purchase the GigE Module using one of the following part numbers:
    • MTT50-28RE
    • MTT50-29RE
    • MTT50-2829RE
  3. Within sixty days of invoice date, complete the following rebate form:

  • Rebate check will automatically be made out to the "Bill To" Information listed on the applicable Sunrise Order / Invoice documentation.
  • Rebate Forms received after sixty days of the Sunrise Invoice date are invalid and will not be honored.
  • The invoice must be paid in full before the rebate is processed.
  • Effective immediately after this rebate is processed, the eligible product from the pull-down menu above will no longer be serviced by Sunrise Telecom.
  • Rebate amounts listed are based on the list price of the GigE Module part numbers.
